Who it's for
Anyone living with a chronic or unexplained condition, people preparing for a specialist visit, and caregivers who track symptoms for someone else.
What's inside
- Daily symptom tracker: pain and fatigue on 1 to 10 scales, sleep hours and quality, energy (spoons) at the start and end of the day, your top three symptoms, medications taken, meals and water, movement, mood and notes
- Weekly symptom summary: pain, fatigue, sleep, energy and top symptom for each day with a weekly average, plus patterns noticed, triggers, what helped and changes to discuss with your provider
- Monthly overview (2 pages): pain, fatigue, flare yes or no and a key note for each of the 31 days, then a summary with averages, flare days, your best and worst day, and the trend against last month
How to use it
- Fill in the daily page at the same time each day. Evenings work for most people.
- Once a week, copy the daily numbers into the weekly summary and note any patterns.
- At month end, finish the overview and bring it to your next appointment.
What to track, and why it helps at appointments
Appointments are short, and symptoms happen all month. A written record of how bad, how often and what you tried turns "it's been bad lately" into dates and numbers your care team can work with.
Track the same few things every day rather than everything once. Pain, fatigue, sleep and your top three symptoms make a good core. Add food, movement and mood if you're looking for triggers.
Look for patterns weekly, not daily. One bad day says little, but three bad days after poor sleep is worth raising. This tracker is an organizer, not medical advice: it doesn't diagnose anything, so bring what you record to your care team.
Questions
Can I use it for any condition?
Yes. The pages are general, so they work for fibromyalgia, POTS, migraine, autoimmune conditions and symptoms that don't have a name yet.
Is this medical advice?
No. It's an organizer. Every diagnosis and treatment decision comes from your care team.
Do I have to sign up to download it?
No. The button gives you the PDF straight away.
