Who it's for
Family caregivers, parents managing a child's medication, and anyone who takes several medications on a schedule.
What's inside
- Medication list: pharmacy, pharmacy phone and date updated, then 13 rows for medicine and strength, dose, when or how often, what it's for, who prescribed it and the date started
- Weekly medication log: medicine and dose, time, and a box for each day from Sunday to Saturday to initial each dose (R for refused, H for held), with notes for missed, refused or held doses
- Monthly medication chart: medicine, dose and time, with a box for each day from 1 to 31
- As-needed medication log: date, time, medicine and dose, reason, given by, and whether it helped
How to use it
- Fill in the medication list first, then copy each medicine into the weekly log or the monthly chart.
- Initial each dose as it's given. If more than one person helps, everyone uses the same sheet.
- Bring the list and the as-needed log to appointments, and update the list whenever a dose changes.
Weekly log or monthly chart?
Use the weekly log when doses are given at set times or more than one person helps: every dose gets initials, and a refused or held dose gets a note. Use the monthly chart for a simple daily routine you just want to tick off.
The as-needed log answers a question doctors often ask: how often a pain or rescue medication was needed, and whether it helped. A few weeks of entries make that conversation much easier.
These sheets are an organizer, not medical advice. Always follow the instructions from the prescriber or pharmacist.
Questions
Can several caregivers share one log?
Yes. Each person initials the doses they give, so everyone can see what has been given and when.
Is it dated?
No. Write in the week or month and print as many copies as you need.
Do I have to sign up to download it?
No. The button gives you the PDF straight away.
